You can set the pressure setting in all aircraft and the atmosphere has the ISA pressure distribution. So QNH does work.
But adjusting the pressure of the atmosphere is currently not simulated.

It gives QNH 1013 hectoPascals in most of the world and 29.92 inches in the U.S.A. That is the ‘Standard Atmosphere’ pressure setting.
