Oof, that looks like a classic floating-point issue where the aircraft's attitude gets completely messed up, leading to the "divide by zero" glitch that Jan mentioned.
Since it seems linked to teleporting or crashing, the best way to avoid it is usually to reset the flight completely instead of teleporting to a new location mid-flight, or just let the simulation settle for a few seconds after a hard reset.
