What is it in the process that tells the sim "this area is flat" when before it was rolling? Where is the "ortho" data changed to make the airport area smooth rather than hilly?

In the TSC file look for the line: <[bool][autoheight][true]>
This parameter is for smoothing the area that the airport sits on.
This data doesn't come from ortho data

If you call a mesh object __airport__runway, aerofly uses a polynom of the 5. degree to flatten spikes and bumps of the elevation grid.
If you call a mesh object __airport__outside, it will still flatten it, but not with the same degree.

We weren't too sure if you meant general smoothing of ground when making an airport or what you call the mesh in AC3D?
My answer means that if you set that parameter to 'true' the objects will sit better on the groundpoly of the airport. Placing objects is a little bit easier as you don't need to be as precise or 'false' will make it harder to place objects flat on the ground because there's no auto leveling. The ground will be more jagged as well but closer to real-world.

I was just asking if you can make a simple mesh around the area of an airport that has smooth ground, with no added runways, objects or cultivation.

interesting question, let's think about it:
We can create a simple mesh and call it ground__airport__outside, so it will 'slightly' smooth the elevation grid.
This mesh is transparent, so the aerial image will be shown through it.
We don't need to add more 3D objects.
But of course this mesh has to be attached to an airport.TSC file, where it is defined.
So we need at least a dummy runway, which will not show up at all. This has been done several times already, for example for the hawaian airports in the download section. So we don't have to create anything else.
If this works, we could copy these files, TSC, TMB and mesh to any position and just edit the parameters like airport name and coordinates to flatten any airport area. So I'm interested to try this with an example. Please tell me an airport you want test this.
Btw, Spit40 does a fantastic job to create a tool for autogenerating airports with meshes, even with runways and surroundings.
As soon as this becomes public, the example described above becomes obsolete.

Do you prefer a specific airport where you currently have some bumps?

I am always trying to create places to fly from to already created airports that represent real routes. Thus in Europe I like to fly to Innsbruck from Frankfort and Vienna in the Q400. Both of them are hilly, Frankfort especially so.
In the US St Louis is a logical spot to fly the the A320 to and from both coasts, but they didn't call St Louis "Mound City" back in the 19th century for nothing!
Places like Philadelphia, Chicago Midway, and Boston are also logical for expanding, but they are reasonably flat already.
